Items Needed During the Registration Process

• Birth certificate
• Medical information:
  • Current immunization records
  • Physical examination within the last 24 months:
    • Physical Examination Form for Pre-K through Grade 6
    • Physical Examination Form for Grades 7-12
  • Red Clay Health History Form (English)
  • Red Clay Health History Form (Spanish)
• Photo ID
• Report card
• Guardianship/custody agreement, if applicable
• IEP documentation, if applicable
• Two forms of proof of residency including at least one item from each group:
  • Group A:
    • A copy of the most recent month’s mortgage statement (if the home was recently purchased and a mortgage statement is not available yet, a copy of a home settlement statement may be accepted in lieu of a mortgage statement)
    • Rental agreement showing the parent, legal guardian or relative caregiver as an occupant
    • Current year's sewer bill
    • Current year's real estate tax receipt
    • A recent original gas or electric bill
  • Group B:
    • Current automobile registration card or automobile insurance policy statement
    • Rental insurance policy statement
    • Current year’s tax documents
    • Paycheck or pay stub dated within the past 30 days
    • Two consecutive bank statements dated within the past 90 days
    • Official US Postal Service Change of Address notification on returned mail (yellow label with new address should be attached to the envelope next to the old address)
    • Correspondence from a State of Delaware agency (e.g., DHSS, DSCYF, Department of Labor, or DSS)
    • If living in someone else's residence:
      • Owner/Renter Affidavit of Multiple Occupancy (English)
      • Owner/Renter Affidavit of Multiple Occupancy (Spanish)
      • Parent/Guardian Affidavit of Multiple Occupancy (English)
      • Parent/Guardian Affidavit of Multiple Occupancy (Spanish)
