Students who want to participate in interscholastic sports a Delaware Interscholastic athletic Association (DIAA) is required. This is different from a regular physical that students get yearly. It is only good for the current school year and must be renewed each year. 

The State of Delware sports year is from April 1st (of the current year) to March 31st (in the same school year).  All pages must be filled out since this form travels with your child to all games and provides valuable information in case your child is injured.
