New Registrations

  • Medical Requirements for Registration 

    New enterers are required to have a physical exam within 2 years of entry.  The physical exam report must be provided for admission to school.

    Lead blood test – Required by Delaware Department of Education for all Kindergarten students (This blood test is usually performed at approximately 12 months of age. If not completed or test results cannot be supplied, the blood test must be completed for entry into Kindergarten).

    4 or more doses of DPT (Diphtheria, Pertussis, Tetanus) vaccine (If child received the 4th dose before 4th birthday, a 5th dose is required)

    3 or more doses of Polio vaccine (If child received the 3rd dose before 4th the birthday, a 4th dose is required)

    2 doses of MMR (Measles, Mumps, Rubella) vaccine

    3 doses of HepB (Hepatitis B) vaccine

    2 doses of Varicella (Chicken Pox) vaccine (All new enterers into

    Kindergarten through 8th grade are required to have 2 doses)

     

    *A disease history/date of Varicella must be verified by a health

    care provider to be exempted from the vaccination*

    Tuberculosis Risk Assessment or (PPD) Tuberculin skin test with results 

     

    Please discuss the following non-required immunizations with your child’s healthcare provider: 

    1. Hepatitis A Series

    2. Meningococcal (Meningitis) - MCV

    3. Human Papillomavirus (for cervical cancer) - HPV

    4. Tetanus & Pertussis booster (Tdap)

    5. Flu vaccine(s)

    6. Pneumoccocal vaccine
